The LPN provides direct nursing care, including medication administration and resident assessments, within a skilled nursing facility. They also coordinate care with the interdisciplinary team and supervise nursing personnel to ensure high-quality, compassionate service.
Candidates must be graduates of an accredited nursing school and hold a current LPN license in the state of practice. A current CPR certification is required, and previous experience in long-term care or skilled nursing is preferred.
Resident Care and Interaction (30%) Provide direct care to residents according to their individualized care plans, ensuring their comfort, privacy, and dignity. Provide exceptional customer service and high-quality care by showing compassion and empathy towards residents and their families. Medication and Treatment Administration (20%) Administer and document physician-ordered medications and treatments accurately and timely. Monitor residents’ responses to medications and treatments, reporting any adverse reactions or concerns. Assessment and Documentation (15%) Observe and monitor residents for changes in condition, and document findings promptly and accurately. Collect and record data for nursing assessments, and initiate or update the nursing plan of care as needed. Coordination and Communication (10%) Maintain consistent communication with the Director of Nursing, physicians, and other members of the healthcare team to coordinate care. Participate in scheduled care plan meetings and consult with interdisciplinary team members as necessary. Compliance and Training (10%) Participate in Quality Assurance reviews and audits to ensure compliance with facility policies and healthcare regulations. Follow infection control guidelines and maintain all privacy rules under the Health Insurance Portability and Accountability Act (HIPAA). Attend all meetings and complete in-services and mandatory training. Supervision and Leadership (5%) Plan, supervise, and evaluate the work of nursing personnel, providing guidance and support as needed. Make rounds regularly and promptly report any changes in residents’ conditions to the appropriate staff. Environmental Safety and Reporting (5%) Maintain a safe, neat, and clean environment for residents and staff. Report any environmental deficiencies, such as lighting issues or equipment problems, and monitor for safety or fire hazards. Other Duties as Assigned (5%) Qualifications: Education: Graduate of an accredited school of nursing.
Current LPN license in the state of practice.
