The Hospice LPN provides direct end-of-life care to patients, including administering medications and monitoring vital signs under RN supervision. They collaborate with an interdisciplinary team to implement care plans and provide emotional support to patients and their families.
Candidates must have completed an accredited LPN program, hold a current state LPN license, and maintain CPR certification. Previous experience in hospice or palliative care is preferred, along with the ability to lift up to 75 pounds and travel within the service area.
Direct Patient Care (40%): Administer medications, treatments, and procedures as prescribed by the physician and under the direction of the RN. Monitor and record patient vital signs and report any changes in patient conditions to the RN or physician. Assist patients with activities of daily living, including bathing, dressing, and grooming. Care Plan Implementation (20%): Follow individualized care plans developed by the interdisciplinary team to meet the physical, emotional, and spiritual needs of hospice patients. Provide input to the RN and interdisciplinary team regarding patient care and progress. Ensure that all care provided is documented accurately and promptly in the patient's medical record. Patient and Family Support (15%): Provide emotional support and education to patients and their families regarding the hospice process and what to expect. Communicate effectively with patients and families to address concerns and ensure they feel supported. Assist families with the preparation for end-of-life care and decision-making. Interdisciplinary Team Collaboration (10%): Participate in interdisciplinary team meetings to discuss patient care plans and progress. Collaborate with other team members, including nurses, physicians, social workers, and chaplains, to provide holistic care. Coordinate with other healthcare providers to ensure continuity of care. Compliance and Training (10%) Follow infection control guidelines and maintain privacy rules and regulations as per HIPAA. Attend all meetings and complete in-services and mandatory training. Provide exceptional customer service and high-quality care by showing compassion and empathy towards patients and their families. Performs other related duties as assigned (5%): QualificationsQualifications: Education: Completion of an accredited Licensed Practical Nurse (LPN) program.
Current LPN licensure in the state of practice. Must possess and maintain current CPR certification.
Previous experience in hospice or palliative care preferred.
