Overview: Under general supervision Care Managers assist in providing rehabilitative, restorative treatment services, crisis intervention, therapeutic intervention, assessment evaluation, and moderately complex, professional-level case management, including visiting with clients in residential or other settings, making referrals and counseling with clients on various issues, and one-on-one behavioral support. This position is based at one of our 24/7 residential programs and requires full schedule flexibility. Candidates must be available to work a variety of shifts, including morning (AM), evening (PM), and overnight hours.

Responsibilities: Demonstrates activities that address the purpose of the Rehabilitative group and individual client plan. Follows the State Mental Health and County Mental Health guidelines for any service being provided. Converses with clients to reinforce positive behaviors and to promote social interaction. Serves meals and eats with clients to act as a role model as it may apply to rehabilitative or behavior plans. Also, menu plans, and grocery shops with clients. Plan and executes regular scheduled activities or outings with the clients for purposes of skill-building. Provide medication support to clients as needed and completes required documentation.

Manager I: An individual who has less than two (2) years of FTE experience in mental health related field providing direct mental health service.

Manager II: An individual who has at least two (2) years but less than four (4) years of FTE experience in a mental health related field providing direct mental health services.

III: An individual who has at least four (4) years of full-time/equivalent (FTE) experience in a mental health-related field providing direct mental health services. Up to two (2) years of the four (4) years of experience can be substituted by years of education in a mental health-related field. There is a minimum requirement of two (2) years of actual work experience.
*The education requirement for a Care Manager III - must be a minimum of two (2) years of education (60 semester or 90 quarter units) with a minimum of 12 semesters (18 quarter) units in a mental health-related subject area such as child development, social work, human behavior, rehabilitation, psychology, or alcohol and drug counseling.
Conditions of Employment
May work across all agency programs as needed. Occasional flexible work schedule required to meet individual client needs. Must possess a valid CA Driver’s License, current auto insurance, and a clean DMV printout (no more than 3 points). Must pass a clear background check with DOJ & FBI. CPR and First Aid Certified. TB Exam and Physical at the time of employment. This position may work within ALL other agency programs, consistent with their scope of practice, on a as needed basis. In addition, may require occasional flexible work schedule to meet the individualized needs of each client. Employees with direct responsibility for children who are under 18 years of age or who are not emancipated from the foster care system and who, in either case, are receiving 24 hour residential care, and employees of 24 hour residential care facilities for the elderly, blind or developmentally disabled individuals may be required to work on-duty meal periods without penalty when necessary to meet regulatory or approved program standards and one of the following two conditions is met: The residential care employees eats with residents during residents' meals and the employer provides the same meal at no charge to the employee; or An employee, except for the night shift, may exercise the right to have an off-duty meal period upon 30 days' notice to the employer for each instance where an off-duty meal is desired, provided that, there shall be no more than one off-duty meal period every two weeks. Driving Requirement: Driving is a required function of this position. Employees must use a personal vehicle that meet all agency requirements, including maintaining liability insurance that meets or exceeds state requirements. Employees are responsible for ensuring the timely renewal of their vehicle insurance policies and providing updated documentation to the agency to remain in good standing and compliant with position requirements. Training Provided: Comprehensive onboarding including CPR/First Aid, Crisis Communication/Pro-ACT, Trauma-Informed Care, Motivational Interviewing, and Collaborative Problem Solving.
