The Community Health Worker will conduct outreach and provide patient education to improve health outcomes within the community. They will collaborate with medical providers and care teams to support patient care plans and assist with navigating healthcare services.
Candidates must have a high school diploma or equivalent, with additional health-related certifications considered a plus. Strong interpersonal skills and familiarity with electronic health records are required, along with a compassionate, patient-focused attitude.
