The provider will deliver direct patient care through office visits and telehealth, including performing assessments, diagnosing conditions, and developing personalized treatment plans. They will also collaborate with a multidisciplinary team and maintain accurate clinical documentation in the EHR.
Candidates must hold an active Oregon license as a Certified Physician Assistant and possess board certification in Family or Internal Medicine. A minimum of two years of clinical experience is preferred, along with a strong interest in functional and lifestyle medicine.
