Provide person-centered assessment, planning, and advocacy for elderly individuals and adults with disabilities enrolled in EW and AC programs. Monitor health and safety needs, develop Coordinated Services and Support Plans, and maintain detailed case documentation.
Requires a Bachelor's degree in Social Work or a related field and experience working with seniors or disabled adults in community settings. A valid Minnesota driver's license and insurance are mandatory for home visits.

A Hiring Bonus of $1000.00 will be awarded to the candidate selected after 30 ($250.00) and 90 days ($750.00) of employment.
A Waiver Case Manager provides person-centered assessment, planning, facilitation, and advocacy for options and services to meet the needs of elderly or adults with disabilities who are enrolled on Elderly Waiver (EW) and Alternative Care (AC) programs.
This position will monitor health and safety needs of individuals and intervene as necessary to ensure effectiveness of services provided and collaborate with other professionals working with those you serve.
This position offers a Hybrid work model where the selected candidate may work in the Office or Remote from home.
