POSITION: 1. Review assessments from referring facilities and note problems/issues that require further attention 2. Contact referral source and other key individuals to gather additional information as necessary and maintain contact with referral source and other key individuals throughout patients treatment 3. Gather comprehensive diagnostic information through psychosocial assessment interview then develop an Integrated Summary, bringing together information from all assessments to identify problem areas that may affect treatment and/or need to be addressed in treatment planning 4. Develop a comprehensive problem list that reflects patients individual issues as described in assessments and the Integrated Summary and present the psychosocial and diagnostic information to treatment team in a clear, concise manner 5. During first session with patient, review initial treatment plan, including case management issues and be sure that patient and staff assignments and responsibilities are understood and in progress 6. Formulate a comprehensive treatment plan with patient that effectively addresses patient issues has clearly stated treatment goals and objectives that are measurable and can be completed within established time frames 7. Participate effectively in multidisciplinary treatment plan reviews, demonstrating adequate preparation and understanding of patient issues, progress, strengths and weaknesses, areas of resistance, positive and negative strategies, and use good clinical judgment in moving patient through treatment levels. 8. Develop interventions that will accomplish goals and objectives and state the planned frequency of interventions 9. Maintain clinical records according to program policies and those of licensing and accrediting agencies, document the patients progress accurately as it relates to treatment goals and objectives, ensure that documentation is timely, consistent, clear and articulate and keep clinical records secure at all times 10. Continue treatment planning process throughout patients stay in treatment, opening new problems and developing or revising treatment plans as necessary 11. Lead and co-facilitate group therapy sessions in a manner that creates an atmosphere in which self- disclosure is facilitated, helps patients gain group process skills, and helps patients gain insight into problems and utilizes experiential techniques effectively 12. Complete activity and group plans for the Clinical Director including supplies utilized and/or needed 13. Conduct educational groups (lectures/videos/discussions) effectively 14. Address problem behaviors, confront counter-productive behavior appropriately in a manner that elicits respect and bonds with the patient while still enforcing program rules and regulations in a firm but fair manner 15. Assist patients to understand treatment goals and give her a clear picture of where she stands with regard to progress toward recovery as seen by the therapist and the multidisciplinary treatment team 16. Adapt to different types of patients 17. Maintain healthy boundaries 18. Understand and apply the programs clinical philosophy 19. Demonstrate good strategic and problem-solving skills 20. Track patients clinical problems in group 21. Provide individual counseling to patients at least one (1) time per week 22. Maintain regular contact and open communication with families, assist families in identifying the problems and issues, conduct at least one (1) weekly family therapy session and assist families in finding resources to help themselves 23. Be effective in finding and linking patient with internal and external resources as needed to meet patient needs 24. Assist patient in managing outside stressors (legal, financial, vocational, etc.) according to the best interests of the patients progress in recovery 25. Prepare patient to maintain recovery post-discharge and help develop a discharge plan that meets patient needs for ongoing support 26. Accept and utilize supervision and address problems noted by supervisor 27. Communicate with supervisor and keep supervisor informed of significant patient developments 28. Participate in Quality Improvement activities and contribute to the welfare of the organization 29. Pursue professional development through participation in education and training programs 30.
