The MDS Coordinator is responsible for conducting and documenting accurate patient assessments and coordinating resident assessment protocols. They also manage the MDS calendar and collaborate with the interdisciplinary team to ensure timely completion of patient care plans.
Candidates must hold a valid Registered Nurse (RN) license and possess excellent communication and organizational skills. Experience in MDS/PDPM and professional certifications like RAC or CMAC are preferred.
