The Community LPN provides direct nursing services, medication administration, and health education to individuals within the community. They work as part of a multidisciplinary team to monitor mental and physical health, perform crisis interventions, and support recovery goals.
Candidates must possess a high school diploma or equivalent and a valid LPN license in the State of New Jersey. Additionally, the role requires at least two years of paid work experience in community nursing and a valid driver's license.

Location: Serving Hudson/Essex Counties
Work hours: Monday-Friday 9:00am-5:00pm
As part of the CSS team, provides nursing services, direct-support services, care management, education and primary health care to persons served. Assures that medical needs are being addressed in the community. Provides support and education to other team members in nursing/medical treatment and health care. Nursing practice supervision is provided by the RN, or in group format quarterly by the Bridgeway Bridge-to-Wellness Medical Advance Practice Nurse.
• High school diploma or equivalent, must have graduated from an accredited nursing training program and possess and maintain licensure, in good standing, in the State of NJ as an LPN.
• One year of technical nursing training supplemented with two years of paid work experience in community nursing
