The Registered Nurse provides comprehensive rehabilitation needs assessments, care management, and direct support services to individuals in the community. They also supervise LPNs, provide health education, and coordinate care with medical professionals to ensure continuity of treatment.
Candidates must be a graduate of an accredited nursing program and hold a valid, good-standing Registered Nurse license in New Jersey. A minimum of one year of community nursing experience and experience with mental health or substance use populations is preferred.

Work hours: Monday-Friday 9:00am-5:00pm (Days/Times Flexible)
As part of the Community Support Services team, provides comprehensive rehabilitation needs assessment and nursing assessment, direct-support services, care management and primary health care to persons served. Assures that the medical needs addressed in the hospital are continued in the community. Provides support and education to other team members in nursing/medical treatment and health care. Nursing practice supervision is provided in group format quarterly by the Bridgeway Bridge-to-Wellness Medical Advance Practice Nurse.
• Must have graduated from an accredited school of nursing program and possess and maintain licensure, in good standing, in the State of NJ as a Registered Nurse. Bachelor’s degree in nursing preferred
