The Registered Nurse will provide skilled nursing care to patients in their homes, including performing assessments and developing individualized care plans. They will also educate patients and families while collaborating with interdisciplinary teams to ensure optimal health outcomes.
Candidates must hold an Associate Degree in Nursing and an active Michigan RN license with at least one year of experience. Additionally, they are required to possess AHA BLS and ACLS certifications, a valid driver's license, and reliable transportation.

The Registered Nurse (RN) will deliver high-quality home health care within a defined local territory (approximately a 30-minute radius). This role focuses on patient-centered care, clinical excellence, and independence in the field.
Provide skilled nursing care to patients in their homes Perform assessments, develop care plans, and monitor patient progress Educate patients and families on care plans and health management Collaborate with interdisciplinary teams to ensure optimal outcomes Maintain accurate and timely documentation
Associate Degree in Nursing (or higher) Active Registered Nurse (RN) license in the State of Michigan Minimum of 1 year of RN experience AHA BLS (Basic Life Support) certification AHA ACLS (Advanced Cardiac Life Support) certification Valid driver’s license, reliable transportation, and auto insurance Willingness to travel within assigned territory
Home Health experience strongly preferred Clean nursing license Experience with adult patient populations (non-pediatric focused)
Strong clinical judgment and independence Excellent communication and patient engagement skills Ability to manage time and workload effectively in a field-based role A compassionate, patient-first mindset
