The Nurse Coordinator will manage referral triage, coordinate complex care for patients, and provide clinical support for the Physical Medicine and Rehabilitation service. They will also deliver patient education and ensure seamless communication between physicians, APPs, and clinical teams.
Candidates must hold a BSN degree and a current California RN license, along with Basic Life Support (BLS) certification. A minimum of three to five years of experience in the field of expertise is required.
