The Hospice Triage RN manages incoming calls from patients and providers to address clinical issues and changes in patient status. They also coordinate physician orders and participate in interdisciplinary team meetings to ensure comprehensive patient care.
Candidates must hold an Associate's Degree from an accredited Registered Nurse program and possess a valid RN license, CPR certification, and a driver's license. One year of hospice experience is preferred, and the role requires up to 20% travel.
