The Palliative Care Social Worker provides psychosocial assessments and evidence-based interventions for patients and families facing serious illness. They also serve as a member of the interdisciplinary team to provide bereavement and grief support across the healthcare system.
A Master's Degree from an accredited School of Social Work is required. Candidates should have at least 2 years of experience in palliative or end-of-life care, with professional licensure in Pennsylvania preferred.
The Palliative Care Social Worker provides psychosocial assessment and evidenced based psycho-social intervention for palliative care patients and their families. A member of the Palliative Care interdisciplinary team, Palliative Care Social Workers utilize their skills and knowledge of person-centered system based clinical skills to alleviate stress associated with serious illness and end of life care. Palliative Care Social Workers provide specialty bereavement and grief interventions to patients, families, and staff across the healthcare system.
Education Master's Degree Accredited School of Social Work Required
Experience 2 years of experience providing Palliative Care or End of Life care Preferred 2 years of social work experience in a medical setting Preferred
