The Memory Care RN provides expert clinical care for patients with neurodegenerative conditions, including medication management and care coordination. They also collaborate with the treatment team to facilitate patient visits and provide education to patients and their families.
The role requires a registered nurse to provide competent direct patient care based on established standards. Candidates should be prepared to assume expanded roles such as charge nurse or preceptor within the ambulatory nursing department.
GENERAL PURPOSE: Under the direction of leadership, is accountable for provision of competent direct patient care based on established standards. Contributes to unit activities and supports enhancement of the care delivery system at the unit level. Supports peers in the delivery of patient care services and assumes an expanded role which may include charge nurse, preceptor, and committee member responsibilities. 40 hours, M-F 8am-4:30pm The Memory Care Program specializes in diagnosing, treating, and caring for neurodegenerative conditions affecting the aging nervous system. The Memory Care RN provides expert care to patients with memory loss, dementia, cognitive dysfunction, and Alzheimer’s disease. Key responsibilities include coordinating care, medication management including refill requests, assisting with prior authorizations, telephone triage, and patient/family education. You will collaborate with the treatment team and ambulatory techs to prepare patient visits and review referrals for new patients to determine next steps.
