The RN Case Manager assumes full nursing responsibility for delivering the Plan of Care for hospice patients. They collaborate with other professionals to implement treatment strategies that promote the physical, psychosocial, emotional, and spiritual well-being of patients.
Candidates must be graduates of an accredited school of nursing and possess at least one year of clinical experience. A valid Georgia RN license, CPR certification, and a valid driver's license with access to a vehicle are required.

Assumes full nursing responsibility for the delivery of the Plan of Care for each hospice patient. Continuously evaluates personal and professional performance and makes necessary changes to increase productivity and quality of care delivered. Adheres to and supports the policies/procedures/goals/objectives of hospice to assure quality patient care. Strives for independence and sound critical decision-making regarding the delivery of patient care in a cost-effective manner without compromising quality. Develops the patient's Plan of care in collaboration with other professionals and implements treatment strategies based on scientific nursing theory that promotes physical, psychosocial, emotional, and spiritual well-being. Maintains patient confidentiality at all times. Familiar with the concepts and needs of patients / families who are facing death and dying. Effective communication skills, both verbally and written.
Education Graduate from a recognized, accredited school of nursing Required Work Experience 1 year of Clinical Experience Required 1 year of Hospice experience Rquired Licenses and Certifications RN - Registered Nurse - Georgia State Licensure and/or NLC/eNCL Multistate Licensure Upon Hire Required CPR - Cardiac Pulmonary Resuscitation Upon Hire Required DL NUMBER - Driver License, Valid and in State Current driver's license, valid vehicle insurance, and access to a dependable vehicle, or public transportation.
