Certified Medical Assistant/Community Health Worker

The Community Health Worker provides outpatient medical assisting services to patients and conducts assessment and referral services to determine the need for services such as care management, appropriate services, or referrals to external resources based on identified needs/concerns, facilitate communication and coordination of services between patients, providers and programs and assist patients in overcoming barriers at the Bowen Health locations as assigned. The individual is responsible for assisting Physicians, Advanced Practice Nurses, Nurses, and Behavioral Health Consultants with delegated responsibilities. Travel between Bowen Health locations may be required.

Duties/Responsibilities

• Below are the primary duties and essential functions of an employee in this position. • Assist physician/nurse practitioner with various clinic duties as assigned, including answering phone calls, assembling medication education materials, and managing the referral process.
• Obtain and enter vital signs and other clinical data in patient record with complete accuracy.
• Prepare patient charts/information for medical appointments to ensure accurate and comprehensive charting.
• Track and coordinate data pertinent to patients' care (i.e.. Medication assistance and sample medications, drug utilization evaluations, etc.).
• Complete prior authorizations for medications and referrals as needed.
• Continue to further education by attending continuing education and training courses for continued certification.
• Review all outpatient medication storage and administration locations for monthly pharmacy review requirements.
• Use standard precautions and OSHA guidelines in the disposal of medical waste.
• Complete and submit billing documents.
• Track and log equipment maintenance documentation.
• Other duties as assigned.
