The Social Worker facilitates discharge planning and conducts psychosocial assessments for medical patients in an acute care setting. They collaborate with an interdisciplinary team to develop and implement effective care plans while providing necessary community referrals.
Candidates must hold a Master's Degree in Social Work and possess 1-3 years of relevant experience. A Licensed Clinical Social Worker (LCSW) license is required within 6 months of employment, along with BLS certification.
Under the direction of the Director of Care Management, the Social Worker provides direct service to clients and their families. The scope of practice includes but is not limited to psychosocial assessments, Discharge planning, and providing referrals for needed services in collaboration with physicians, evaluates and implements treatment regime and discharge plans for designated medical patient populations.
