We are seeking a dedicated Family Program Clinical Case Manager to support residents and families in our transitional housing program. This role provides a combination of case management, counseling, and recovery support, helping families achieve stability through individualized planning, community connections, and ongoing therapeutic engagement. You'll play a key role in guiding families through their recovery journey-supporting mental health, housing transitions, and long-term independence. Key Responsibilities Facilitate new resident orientation, including move-in support and development of Individual Recovery Plans (IRPs) Provide weekly counseling and case management to families, tracking progress toward goals Serve as a therapist, advocate, and care coordinator, connecting residents to: Mental health services Substance use treatment Healthcare and community resources Support residents in securing permanent housing upon program completion Monitor and coordinate drug screening for assigned residents Maintain accurate, confidential documentation in compliance with agency standards Collaborate with internal teams (program staff, employment, education, and intake) Participate in case reviews, staff meetings, and supervision Contribute to a team-based environment, including occasional on-call support
Master's degree in Social Work, Counseling, Psychology, Family Therapy, or related field 3-5 years of counseling and/or case management experience Active Maryland licensure (LMSW, LGPC, or equivalent) Experience with: Trauma-informed care Substance use and co-occurring disorders Evidence-based approaches (e.g., Motivational Interviewing, CBT) Strong organizational, documentation, and communication skills Ability to build therapeutic relationships and work collaboratively Valid driver's license and reliable transportation Bonus: We offer in-house clinical supervision for those pursuing LCSW-C or LCPC licensure.
