The Care Manager II provides person-centered care coordination for high-need Medi-Cal members, focusing on medical, behavioral, and social needs. They are responsible for creating housing plans, conducting in-person client visits, and acting as a central hub of communication between patients, families, and healthcare providers.
Candidates must hold a Master’s degree in Social Work (MSW) and possess at least 2 years of direct Enhanced Care Management experience. Proficiency in social determinants of health, community resources, and database management is required, with Russian language skills preferred.

We are looking for a Care Manager II (CMII) to join our team under the California Advancing and Innovating Medi-Cal (CalAIM). The ideal candidate is responsible for helping clients navigate the difficult housing situation in Santa Clara county and will report to a Clinical Supervisor.
Requirements: Key Responsibilities:
Act as the central hub of communication, ensuring a smooth exchange of information among patients, families, healthcare providers, and all involved parties.
Education: Master’s degree in Social Work (MSW) required Experience: Minimum 2 years of direct Enhanced Care Management (ECM) experience required Strong knowledge of social determinants of health and community resources Familiarity with Medi-Cal, CalAIM, and the 14 Community Supports services (preferred) Experience working with databases, specifically Apricot, is highly desirable. Cultural competence and sensitivity to diverse populations Motivational Interviewing Needs assessment and care planning Skills and Competencies: Excellent interpersonal and communication skills. Service coordination and navigational skills Strong organizational and time management abilities. Proficiency in data entry and database management. Ability to work independently and as part of a team. Knowledge of the local community and available resources. Russian-speaking candidates preferred Salary and Benefits:
$80,000 - $90,000 annually. Full-Time, Monday to Friday.
This position is full-time, 37.5 hours/week. The salary is commensurate with the selected candidate's qualifications and experience.
