The Housing Coordinator identifies and secures appropriate community-based housing for clients transitioning from institutional care. They coordinate resources, facilitate rental assistance applications, and manage accessibility modifications to ensure safe transitions.
A Bachelor's degree is required, though three years of direct real estate experience may be substituted. Candidates should possess knowledge of Connecticut landlord/tenant regulations and experience working with diverse populations.
The Housing Coordinator will serve as a key member of the Money Follows the Person (MFP) Team, supporting clients in their transition from institutional care to community-based living. This position focuses on identifying housing opportunities, coordinating resources, and ensuring clients have the tools needed for a safe and successful move.
