The Care Manager develops, implements, and monitors individualized, person-centered care plans to support clients in living safely and independently. They also collaborate with healthcare providers and community partners to coordinate essential services and ensure ongoing compliance with program guidelines.
Candidates must hold a bachelor's degree in Social Work, Nursing, Psychology, or a related human services field. At least two years of experience in care or case management is required, along with a valid driver's license.

The Community First Choice (CFC) Care Manager plays a vital role in supporting individuals who need assistance to live safely and independently in their homes and communities. This position is responsible for developing, implementing, and monitoring individualized, person-centered care plans that follow CFC program guidelines and promote dignity, autonomy, and successful community living.
The Care Manager collaborates with clients, families, healthcare providers, and community partners to coordinate access to essential services. Through ongoing assessment and monitoring, the Care Manager ensures that care plans remain responsive as client needs change. This position directly enhances quality of life for older adults and individuals with disabilities by fostering independence and community integration.
