The Care Coordinator provides therapeutic case management, crisis intervention, and rehabilitation services to chronically homeless individuals. They are responsible for developing person-centered service plans, conducting outreach, and maintaining accurate documentation in compliance with regulatory standards.
A high school diploma is required, along with four years of paid work experience in mental health or social services. Candidates must possess a valid California driver’s license, a personal vehicle, and proof of insurance for extensive local travel.
The Care Coordinator is responsible for developing and providing high quality therapeutic interventions and services in support of an individual’s wellness and recovery. Services include strength-based and needs-based therapeutic case management, problem-solving, therapeutic rehabilitation including skills-building, benefits support, resource development, housing navigation, crisis intervention, and family support.
This is a community-based position providing direct, face-to-face services to individuals who are chronically homeless and travel throughout Sacramento county is required.
