The Home Health Liaison facilitates patient transitions by coordinating discharge planning and home care services. The role involves working on-site at the clinic to ensure continuity of care for patients.
Candidates must be a graduate of an accredited School of Nursing and hold a valid RN license and driver's license. A BSN is preferred, along with at least one year of professional nursing experience in an acute care setting.
Graduate of an accredited School of Nursing/College. Required BSN. Preferred Work Experience At least one year professional nurse experience within the last five years in an acute care setting. Required One year of experience in home care and hospital discharge planning. Preferred Licenses & Certifications Valid RN license. Required Valid State driver’s license.
