The Social Worker will develop detailed social histories and individualized care plans to support the emotional and social well-being of residents. They will also coordinate discharge planning and facilitate staff training while building relationships with community partners.
Candidates must hold a Bachelor’s Degree in Social Work or a Social Worker Designee certification. Experience in long-term care settings and strong interpersonal communication skills are required for this role.
