The Case Management Coordinator assesses individual needs, develops strength-based treatment plans, and provides ongoing support to clients. They also manage clinical documentation, facilitate referrals, and advocate for community resources to assist individuals in their recovery journey.
Candidates must be a Qualified Mental Health Professional (QMHP) as defined by Medicaid guidelines. A bachelor's degree in a human services field and at least one year of experience working with individuals with mental illness are required.

• Must be a QMHP in accordance with Medicaid Provider Manual Guidelines. 1. Possess specialized training (including fieldwork and/or internships associated with the academic curriculum where the student works directly with persons receiving mental health services as part of that experience) OR one year of experience in treating or working with a person who has mental illness; AND 2. Be a human services professional with at least a bachelor’s degree in a human services field
