Registered Nurse RN Case Manager Home Health Lackawanna County, PA (Scranton, Dunmore, Taylor, Moosic) Interim HealthCare of Scranton $38–$40/hour + $5,000 Sign-On Bonus + Weekly Pay

Immediate opening for a Registered Nurse RN Case Manager Home Health in Lackawanna County, PA including Scranton, Dunmore, Taylor, and Moosic. Manage your own caseload, provide one-on-one patient care, and avoid hospital burnout while working in the community.

$5,000 sign-on bonus

Weekly pay with direct deposit

Mileage reimbursement

SCHEDULE • Monday through Friday 8

On-call, weekend, and holiday rotation

Registered nurse rn case manager responsibilities
Manage a home health caseload in Lackawanna County
Perform RN assessments and reassessments
Develop and update care plans
Coordinate care with physicians and healthcare team
Complete home health documentation and ensure compliance
Educate patients and families

Active Pennsylvania Registered Nurse RN license
Minimum 1 year home health RN experience
Case Manager or Case Management experience preferred
CPR certification
Valid driver’s license and reliable transportation
