The Nurse Practitioner will facilitate efforts to relieve suffering and enhance quality of life for patients with serious illnesses through holistic assessment and symptom management. They will collaborate with the transdisciplinary team to develop and implement care plans while providing expert consultation and education to staff and families.
Candidates must hold a Master of Nursing degree, a current Registered Nurse license in Ohio, and a Certificate of Authority. A minimum of two years of clinical experience is required, along with the ability to provide own transportation for patient visits.

The Nurse Practitioner will facilitate HWR’s efforts to relieve suffering, enhance comfort, promote quality of life and foster choice in the presence of an advanced serious illness for all patients and families. The practice is based on competent clinical knowledge and focuses on clinical practice, education, collaboration, consultation and research related to palliative care.
This is medium to heavy work requiring the exertion of up to 75 pounds of force occasionally and up to 20 pounds of force frequently, and the use of appropriate safety transfer techniques and body mechanics when moving, transporting, etc. of patients, sometimes in excess of 100 pounds; work requires stooping, reaching, standing, walking, lifting, fingering, grasping, and feeling; vocal communication; hearing; visual acuity and color perception. Employee understands and accepts possibility of exposure to environmental elements, internal and external, such as infectious disease, bloodborne pathogens, and chemicals and/or chemical fumes, and adverse effects of inclement weather that may occur.
