Provide comprehensive primary care services to geriatric patients in an outpatient clinic setting. Coordinate with on-site cardiology, imaging, and nutritional services to ensure holistic patient health.
Must be an experienced Nurse Practitioner with a focus on geriatric care. Ability to participate in a shared phone-based call rotation is required.
