The Preventative Care Coordinator identifies and engages patients overdue for care to close health gaps and improve clinical outcomes. This role involves conducting patient outreach, scheduling appointments, and documenting activities within the electronic health record system.
A high school diploma is required, with a college degree in a related field preferred. Candidates should have experience in healthcare, specifically in care coordination or patient outreach, and proficiency with electronic medical records and Microsoft Office.
Under the supervision of the Quality Manager, the preventive care coordinator will play a vital role in supporting the Neighborhood Health’s quality improvement initiatives by identifying and engaging patients who are overdue for care. This position is responsible for outreach to patients, scheduling necessary appointments, providing education on the importance of preventive and chronic care services, and ensuring follow-through to close care gaps. The role supports improved health outcomes, patient satisfaction, and compliance with value-based care metrics. Data and outreach will be reported out to the Neighborhood Health Quality Committee, and Neighborhood Health Leadership team.
