Provide primary and preventative medical care to geriatric patients across various long-term care facilities. Manage chronic conditions and coordinate care with interdisciplinary teams, families, and specialists.
Must hold an active Florida ARNP license and be Board Certified (ANCC/AANP). Experience in geriatrics, internal medicine, or long-term care is preferred, along with reliable transportation for travel.
