The RN Case Manager is responsible for leading the coordination of compassionate, patient-centered care plans for hospice patients. They will build meaningful connections with patients and families while providing care in the field.
Candidates must hold an active Colorado Nursing License. At least one year of home health or hospice experience is preferred.
