The Health Care Navigator provides outreach, enrollment, and retention services to individuals eligible for government-sponsored insurance programs. They are responsible for meeting enrollment goals and collaborating with community partners to generate leads.
Candidates must have a High School Diploma/GED with experience or an Associate's degree in a related healthcare field. Additionally, the role requires completion of the NYS Department of Health training program and certification.

CAP is looking for a Health Care Navigator. The Health Care Navigator (HCN) provides outreach, enrollment, and retention services to families and individuals eligible for government sponsored insurance programs. The HCN is responsible for meeting projected enrollment goals, coordinating outreach and enrollment efforts by working collaboratively with other staff, member clinics and community-based organizations. The HCN will work independently and have the ability to network and self-generate leads.
This is a full-time, non-exempt position scheduled for 35 hrs/wk, with an hourly rate of $20.41.
