The Direct Support Professional provides personal care and support services to individuals with disabilities or chronic illnesses to help them live independently. Responsibilities include assisting with daily living activities, medication monitoring, implementing behavior plans, and maintaining accurate service documentation.
Candidates should have a high school diploma or GED, with preference given to those with human services training such as CNA or Home Health Aide certification. Applicants must be at least 21 years old, possess a valid driver's license, and be able to pass a criminal background check.
This position is responsible for providing personal care services and other support to individuals with disabilities, elderly, or chronic illness to enhance their well-being and skills needed to live on their own. This includes, but is not limited to, providing assistance and training activities of daily living, personal care services, and job-related supports. Services are provided primarily in the residence of the person(s) served in a variety of independent living settings. Completion of training, timesheets, progress notes/service delivery logs, and other paperwork for billing and licensing are required.

Physical requirements include sitting, standing, climbing, pushing, bending, squatting, lifting, and pulling. Employee may be required to provide mobility assistance to consumers weighing up to 300 lbs.
