Support older adults in the PACE program by assisting with daily activities such as bathing, dressing, and mobility. Monitor vital signs and collaborate with an interdisciplinary team to ensure participant safety and independence.
Requires a high school diploma and CNA or HHA certification, with at least one year of direct care experience for older adults. Must possess current CPR and First Aid certifications and the ability to pass background checks.
