The Care Manager provides direct behavioral health and case management services to adults and families with complex needs. Responsibilities include conducting screenings, crisis support, leading psychoeducational groups, and assisting clients with daily living activities.
A bachelor's degree in Social Work, Psychology, or a related field is required, along with a minimum of one year of relevant experience in behavioral health or case management. Experience with co-occurring conditions and housing navigation is preferred.
ACT Care Manager - Adams/Denver County

We offer a wide range of services, including step-down care from inpatient hospitalization for youth, comprehensive support for families, and specialized treatment for youth facing depression, trauma, behavioral challenges, and family conflict.
Our adult programming includes intensive care management, Assertive Community Treatment (ACT), integrated co-occurring IOP, individual and family therapy, medication management (including MAT), case management and peer support, supported employment/housing, and recovery services for mental health and substance use disorders
This position is based in Adams/Denver County and requires traveling to client homes and within communities. The Care Manager provides direct behavioral health and case management services to adults and families with complex needs, including individuals served through ACT-informed practices. This position will also provide outpatient competency restoration education for individuals that have been deemed incompetent by the court.
Schedule Required to provide 2–3 evenings per week between 4–8 pm to support client availability. Occasional weekend or crisis coverage as needed for team coverage.
Housing Assist clients with navigating housing systems, including completing applications for subsidized housing, transitional housing, or supportive housing programs. Collaborate with team members to identify barriers to housing, such as credit issues, criminal history, or lack of documentation, and develop plans to address them. Provide accompaniment to housing appointments or inspections, when necessary, to support client self-advocacy and engagement.
Education Bachelor's degree in Social Work, Psychology, or a related behavioral health field (required).
Experience Minimum one year of relevant experience in behavioral health and/or case management (required). Experience working with individuals with co-occurring mental health and substance use conditions and/or ACT teams is strongly preferred. Experience with housing navigation is a plus Spanish-speaking/bilingual candidates preferred.
Additional Requirements Valid driver’s license and auto insurance. Must live in or near the community served.
Position Details: Job Type: Full-time, hourly paid
$26.44 – $31.25/hour (equivalent to $55,000 – $65,000 annually)
