The Clinical Care Coordinator manages the client intake and admissions process while overseeing case management, aftercare planning, and discharge. They also interface with referents, family members, and the clinical team to ensure smooth day-to-day facility operations.
Candidates must hold a high school diploma or GED, with an associate or bachelor's degree preferred, and possess a valid RADC, CADC, CATC, or SUDCC certification. Experience with dually diagnosed populations and proficiency in electronic health record systems and insurance utilization reviews is required.
