The coordinator performs health and functional assessments to develop individualized care plans for older adults and individuals with disabilities. They monitor progress through regular contact and home visits to ensure members remain independent and safe in their communities.
Candidates should have at least two years of experience in human services, preferably with the elderly, and a bachelor's degree in social work or a related field. Proficiency in Microsoft Office and bilingual skills are highly preferred.
