The Community Health Worker provides outreach and navigation services to individuals experiencing homelessness to connect them with medical and behavioral health care. They are responsible for assessing patient needs, creating care plans, and facilitating access to essential community resources.
Candidates must have a high school diploma or GED and at least two years of relevant work or internship experience. A valid Massachusetts driver's license and bilingual proficiency are required for this role.
The Community Health Worker (CHW)/Patient Navigator plays a key role in delivering outreach, navigation, and support services to patients of Worcester Health Care for the Homeless, which is located at 199 Chandler Street, Worcester. This position focuses on building trusting relationships with individuals experiencing homelessness, connecting them to medical and behavioral health care, and supporting their progress toward stability and improved overall well-being.
