This role involves providing professional diabetes and foot care services, including patient education on management, assisting with care plans, and conducting comprehensive foot assessments under clinic supervision. The educator will also develop educational programs, advise on complications, and coordinate with the healthcare team for optimal patient care.
Candidates must possess an Associate's degree or higher in Nursing, hold a current Michigan RN license, and have two or more years of experience in diabetic care, wound management, or foot care nursing. Essential qualifications also include being a Certified Diabetic Educator (CDE), holding a Diabetic Foot Care certificate, and possessing a valid driver's license.

Diabetes Educator/Foot Care RN - Health Services Dual Site Indian Preference applies in accordance with Ordinance #15-600-02
Provides exceptional customer service while receiving and assisting patients. Assists providers in developing and administering patient care plans. Provides evidence-based education related to diabetes and the disease process; medication, diet, physical activity and nutritional monitoring; foot care, prevention and care of complications; blood glucose monitoring and the techniques of self-monitoring and injection. Develops and implement educational programs for diabetic patients and families. Advises patients on long term complications. Provides diabetic resources and consultation to the clinical team. Assesses, monitors and implements treatment plans for patients at various stages of the disease; conducts regular follow-ups with patients. Conduct comprehensive foot assessments for patients with diabetes, identifying potential risks such as neuropathy, ulcers, or circulation issues. Provide safe and compassionate foot care, including nail trimming, skin care and management of corns, calluses and ingrown nails. Document assessment, interventions and plans including recommendations and goals, progress notes and discharge summaries. Maintains strict adherence to infection prevention and control practices, including proper instrument cleaning and sterilization. Develops program goals and objectives, reports and manages Tribal specific data related to the diabetic registration for internal planning Coordinates with other tribal and community services to enhance access of services and resources. Maintains professionalism and confidentiality in all aspects of work and complies with all applicable tribal, federal, or state laws and procedures. All other position/department related duties as assigned by supervisor.
