$35 / HOUR
Provide high-quality, patient-centered nursing care in an ambulatory primary care setting, including nursing assessments, triage, and direct patient care. Coordinate continuity of care following hospital discharges and support chronic disease management through patient education.
Requires a current Registered Nurse (RN) license in Connecticut and CPR/BLS certification. A Bachelor of Science in Nursing (BSN) and experience in ambulatory or behavioral health settings are preferred.
