The Support Coordinator ensures continuity of care for children and adolescents with intellectual and developmental disabilities by developing person-centered plans. They link clients with service providers, monitor progress, and advocate for medical, educational, and vocational needs.
Candidates must possess a Bachelor's degree in Human Services (such as Social Work, Psychology, or Counseling) and one year of experience in support coordination. A valid Virginia Driver's License and the ability to pass comprehensive background checks are also required.
