The Case Manager provides education on the Medicaid waiver process and creates individualized support plans based on client needs. They facilitate community-based team meetings and help individuals access necessary services and supports.
Candidates must have a Bachelor's Degree in Human Services and at least one year of professional experience working with individuals with intellectual and developmental disabilities. A valid driver's license, reliable transportation, and a designated home office setup are required.
