The Licensed Practical Nurse (per diem) works under the direction of a registered nurse to provide nursing care, to assist the patient in learning self-care techniques and to prepare clinical and progress notes.

Responsibilities
- 1. Assesses patients within scope of practice at every home visit. 2. Carries out treatments per MD orders. Reports off to RN case manager and/or clinical manager. 3. Reports all changes in condition to MD. 4. Provides patient/caregiver education as per care plan. 5. Completes all documentation within 24 hours. Must include changes in condition, follow-up with provider or other disciplines with supporting documentation. 6. Attends all staff meetings, inservices, and all other meetings as required. 7. Compliance with all mandatory requirements
- inclusive of Healthstreams. 8. Follows policies and procedures.
Qualifications
- EDUCATION Graduated from an approved Practical Nurses Education Program EXPERIENCE Minimum one year experience in the acute care setting.
- Home care experience preferred but not required.
- LICENSURE License Practical Nurse with valid (or eligible) Connecticut license required.
- A valid driver's license and proof of car insurance are required when using a personal vehicle for company business.
- Access to a dependable vehicle in order to travel to multiple stops a day.
- CPR certification is required and must be maintained.
- Candidates who do not have certification are required to obtain it within 3 months (90 days) of hire.
- SPECIAL SKILLS Excellent verbal and written skills.
- Excellent clinical knowledge.
- Strong computer skills.
- PHYSICAL DEMAND Ability to perform tasks involving physical activity, which includes heavy lifting, extensive bending, administering CPR, patient bracing, standing, and stairs.
- Requires exposure to blood and body fluids, hazardous materials, and communicable diseases.
