Provide comprehensive healthcare and coordinate care for patients with endocrine disorders in skilled nursing and long-term care facilities. Responsibilities include conducting assessments, ordering diagnostic tests, and traveling to various facilities within the region.
Requires a Master's degree in Nursing, a valid NP, PA, or Physician license, and DEA certification. Candidates must have at least one year of experience in Endocrinology and six months in long-term care settings.
Provide comprehensive healthcare services and coordinate patient care Assessing and treating patients with endocrinology diseases and conditions Work closely with a team of healthcare professionals Conduct thorough patient assessments, including medical history reviews and physical examinations Order and interpret diagnostic tests Develop accurate diagnoses and treatment plans Travel regularly to multiple nursing homes within the designated region to deliver on-site care Maintain a flexible schedule to accommodate facility visits and optimize patient coverage
Minimum 6 months of experience in long-term care or a skilled nursing facility Active and unrestricted Nurse Practitioner, Physician Assistant, or Physician license DEA License CPR BCLS/BLS certified Must hold a Master's Degree in Nursing Completed a degree from an accredited Nurse Practitioner program A year of experience in Endocrinology Preferred experience in Documenting EMR (Sigma, Visual, PCC, etc.): training provided Must have great organizational and time management skills. Reliable transportation Must be willing to travel to different nursing homes within the state of Pennsylvania (100% commitment) Must be able to lift a minimum of 20 pounds and stand / walk / work on your feet up to 6-8 hours per day
