Your Impact As a Home Health RN, you’ll be the heart of patient care, bringing clinical expertise and compassion directly into the home.
This role combines Start of Care (SOC), wound care, and infusion therapy responsibilities, providing high-quality nursing care to patients in their homes.
You will: Perform Start of Care (SOC) patient visits, including initial physical assessment, medication reconciliation, home safety evaluations Create personalized care plans to improve patient outcomes and support their independence Perform wound assessments and provide wound care treatments if necessary Administer and monitor IV/infusion therapies per physician orders when required Complete accurate and timely clinical OASIS documentation Act as a trusted Case Manager, coordinating care with physicians, therapists, and the interdisciplinary team Educate and empower patients and caregivers with confidence Our Commitment to You Flexible schedule to fit your life and support balance Ongoing professional development and training A truly collaborative team that celebrates your success Our Requirements Currently licensed as a Registered Nurse (RN) in the State of Arizona Minimum of 1 year Home Health preferred Wound care and/or infusion experience preferred Strong clinical skills and a passion for patient-centered care Expert organizational and time management skills Current CPR certification Valid AZ driver’s license, proof of auto insurance, and reliable transportation Benefits Medical / Dental / Vision Insurance Paid Time Off 401(k) matching Employee Assistance Program FSA / HSA Life Insurance Employee Referral Program The employer for this position is stated in the job posting.
