We are seeking an experienced and compassionate Hospice & Palliative Care Nurse Practitioner (NP) to join our clinical team on a part-time basis. In this role, you will provide advanced medical care to hospice and palliative patients in the home setting, functioning independently while collaborating with physicians as required by state regulations. This position is ideal for a clinician who values autonomy, clinical excellence, and meaningful patient relationships.
Conduct Face-to-Face (F2F) encounters and report findings to the certifying physician.
Provide comprehensive medical care and write appropriate orders for hospice and palliative patients.
Manage symptom control and pain management to optimize patient comfort and dignity.
Serve as the attending provider when designated by the patient (excluding certification of terminal illness).
Communicate effectively with patients, families, physicians, and interdisciplinary team members.
Maintain accurate, timely, and compliant clinical documentation per hospice policy.
Supervise and provide clinical guidance to LVNs and HCAs as appropriate.
Participate in case conferences, team meetings, and care planning discussions.
Bachelor of Science in Nursing and NP-focused Master’s or Doctoral degree.
Current and valid APRN license in the State of New Mexico.
National Nurse Practitioner Board Certification.
Minimum of 3 years of nursing experience with expertise in symptom management and pain control.
Knowledge of hospice, palliative care principles, and psychosocial aspects of terminal illness.
Valid driver’s license and current automobile liability insurance.
Not listed on the OIG Excluded Providers list.
