Provide acute and chronic care management for adult and geriatric patients in Plymouth and Sheboygan. Collaborate with physicians and specialty providers to develop and implement comprehensive treatment plans.
Must hold an Advanced Practice Provider license as a Nurse Practitioner or Physician Assistant with a valid DEA and BLS certification. Requires strong diagnostic skills and the ability to work independently within interdisciplinary teams.
Nurse Practitioner/Physician Assistant Summary: Full-Time | Monday-Friday only
This role is focused on providing both acute and chronic care management for adult and geriatric patients in our Plymouth and Sheboygan service areas. The Nurse Practitioner or Physician Assistant collaborates with supervising physicians to develop and implement comprehensive treatment plans that improve patient outcomes and quality of life.
