The Care Coordinator helps individuals and families develop skills and access resources to manage illness and promote recovery. Key duties include providing education on mental health and substance use, linking clients to community resources, and maintaining clinical documentation.
Candidates must have a degree in human services, a QAP certification, or a combination of a non-human services degree and qualifying experience. Experience must include delivering services to individuals with mental illness, substance use disorders, or developmental disabilities.
The services provided by a Care Coordinator (CC) focus on helping individuals and family develop skills, access resources and manage illness to be successful in the living, working, learning and social environments of their choice. CC’s teach, model and practice skills with person(s)-served to promote recovery and wellness.
