Provide quality care and condition education to Medicaid members through home visits and phone interactions. Coordinate transitions from care facilities to homes and connect members with community resources.
Requires an independent clinical license and a Master's degree in Psychology, Social Work, or a related behavioral health field. Must have at least 2 years of post-licensure experience and reside near Athens, OH.
Independently licensed clinician, with a Master's degree in Psychology, Social Work, Counseling, or related behavioral health discipline Licenses must be independent, active, and unrestricted in the state of Ohio 2+ years of post-licensure experience in a related mental health environment Proficient computer skills and good working knowledge of Microsoft Word and Outlook Access to high-speed internet and a dedicated, distraction-free workspace at home Reliable transportation and the ability to travel within the Athens, OH area Reside in or near Athens, OH
Experience working with the Medicaid population Community Mental Health experience Experience working in an environment that required coordination of benefits and utilization of multiple groups and resources for patients Experience working with low-income populations Experience working with either Medicaid or Medicare funded programs Medical / Behavioral setting experience (i.e., hospital, managed care organization, or joint medical / behavioral outpatient practice) Dual diagnosis experience with mental health and substance abuse Clinical training experience *All employees working remotely will be required to adhere to UnitedHealth Group’s Telecommuter Policy.
